if you are to both play and record the game at the same time, i think the best option still is to use pen and paper.
kifu paper is available online, very helpful.
after finishing the game, make a .sgf file from the kifu.

I like using pen and paper, but I can't really say that its the best.
Electronic recording apps make it so much easier, and far less distracting, not having to remember move numbers and such.
I only use paper mostly because I can't afford the apps/devices.

Tossing another vote in for Gobandroid. It's really great and comes with some nice extras like problems and pre-recorded SGFs.

A lot of digital clocks will display and increment the move number for you as moves are made. Of course, this doesn't help when your opponent fails to punch their clock and you punch yours without noticing.
