Would white simply play Q18?

r18 is much better shape. Then you can aim for n17 and q14.

I remember Dinerstein played it in his famous win against O Rissei 9p.

It fixes black's top while you'll have to come back to fix the weakness at "a".

Basically I think you should only ever play q18 if you intend to immediately take gote to play s18 afterwards. That will hopefully make you more reluctant to make that "fake sente" q18 exchange.
